Reducing the variability in apparent operative inclination during total hip arthroplasty. Findings of a prospective randomised controlled trial

Abstract

Aims: To determine during total hip arthroplasty (THA) which of three methods best achieved a target operative inclination of 35o +/- 2.5°: 1. Freehand, 2. Modified Mechanical Alignment Guide (MAG), 3. Digital Inclinometer. Patients and Methods: 270 participants were recruited with 90 randomised to each group. All had a cementless Pinnacle® acetabular cup implanted via a posterior approach in lateral decubitus. The primary outcome was the unsigned deviation from target operative inclination. The digital inclinometer was used to measure operative inclination in all cases, though the surgeon remained blinded to the reading intra-operatively for both the Freehand and MAG methods. Results: With respect to our target range of 35 o ± 2.5o. This was achieved with the digital inclinometer in 88% of cases as compared to 71% and 51% with the MAG and freehand techniques respectively. The differences between each were significant (p=0.006 and p<0.001). The maximum error for each method was 7o, 6o and 10 o respectively. Conclusion and clinical relevance: There are simple methods other than freehand that can allow the surgeon to reduce the error in achieving a target for operative inclination. However, translating that into a target postoperative radiographic inclination is also dependent on operative anteversion and critically pelvic position.
