Abstract

We model the service level decision for multi-morbid chronic patients using an MDP framework. For chronic diseases, patient behaviour and engagement is an important factor that affects health outcomes and healthcare costs. We model patient state on two dimensions, patient disease complexity and patient activation level. We study admission policies for a complex care system under parameter uncertainty.
