Abstract

An error was discovered in the Materials sub-section of the Methods section. The fifth sentence should read "Each question was scored as 'yes' = 2, 'impossible to decide' = 1, and 'no' = 0, and the individual item scores were summated." The results and conclusions are unaffected.The authors regret this error.
