Description
"Social prescribing is a new and exciting development within the Social Care Sector, involving service users in a number of activities that improve physical and mental wellbeing whilst simultaneously developing community resources which promote independence, resilience and the sharing of skills. Examples of social prescriptions include learning groups, activity sessions, exercising, volunteering and the use of community resources.
This project develops the concept of social prescribing into an integrated service user-focused system using data from a number of available sources. Using up to date technology, including smart-phone apps and web services our solution helps service users connect with available resources in their locality and assist our health professionals make appropriate referrals. Successful completion of this project will produce an exciting, secure and scalable product which can be rolled-out in other parts of the country, helping people to remain independent and healthy, freeing up appointments in GP practices and hospitals and finding cost-savings for the NHS."
